SENATE CONCURRENT

RESOLUTION

 

 

Requesting the ESTABLISHMENT of an advisory committee to make recommendations regarding the creation of a hawaii all-payer health claims database for the purpose of transparent public reporting of health care information.

 

 


     WHEREAS, according to the State Scorecard published in 2007 by the Commonwealth Fund, Hawaii ranked number one among the states in health system performance and is a leader in improving and reforming its health system; and

 

     WHEREAS, Hawaii is always seeking to improve the quality of health care provided; and

 

     WHEREAS, all-payer claims databases provide information necessary to determine utilization patterns and rates, identify gaps in needed disease prevention and health promotion, assist with benefit design and planning, establish clinical guideline measurements related to quality, safety, and continuity of care; and

 

     WHEREAS, seven states have already established all-payer claims databases and three more states are currently developing programs; and

 

     WHEREAS, all-payer claims databases provide benefits and information to consumers, employers, health plans, insurance providers, researchers, and state government; and

 

     WHEREAS, all-payer claims databases increase insurers', hospitals', and physicians' accountability by providing consumers with easily accessed and understood information comparing services, costs, and success rates; and

 

     WHEREAS, all-payer claims databases will provide the State and hospitals with the comprehensive information necessary to reform Hawaii's hospitals to be cost-efficient without sacrificing their high standard of care; now, therefore,

 

     BE IT RESOLVED by the Senate of the Twenty-fifth Legislature of the State of Hawaii, Regular Session of 2010, the House of Representatives concurring, that the Insurance Commissioner is requested to establish an advisory committee to make recommendations regarding the creation and maintenance of a Hawaii all-payer claims database for the purpose of publicly reporting health care and health quality data including information on safety, quality, cost, and efficiency at all levels of health care; and

 

     BE IT FURTHER RESOLVED that the advisory committee shall be named the Committee on the Creation of a Hawaii All-Payer Health Claims Database (Committee); and

 

     BE IT FURTHER RESOLVED that the purpose of the Committee is to research and report to the Legislature:

 

     (1)  What data is necessary to measure the safety, timeliness, effectiveness, efficiency, equity, and patient-centeredness of Hawaii's health care;

 

     (2)  Specific methods and strategies, including mandatory and voluntary reporting, to measure and collect data related to health care safety and quality, utilization, cost to payers and purchasers, and health outcomes;

 

     (3)  Recommendations about the inclusion of data on the uninsured;

 

     (4)  Types of information that allow public and private health care purchasers, consumers, and data analysts to identify and compare health plans, health insurers, health care facilities, and health care providers regarding the provision of safe, cost-effective, high-quality health care services;

 

     (5)  Data elements that foster quality improvement and peer group comparisons;

 

     (6)  Strategies to facilitate value-based, cost-effective purchasing of health care services by public and private purchasers and consumers;

 

     (7)  Methods to use and build upon existing data collection standards and methods to establish and maintain the database in a cost-effective and efficient manner;

 

     (8)  Recommendations on where the database shall be located;

 

     (9)  Discussion on whether to harmonize a Hawaii database with other states' and regions' all-payer claims databases and what steps would be necessary to do so; and

 

    (10)  Viable options of funding the creation of the all-payer health claims database and a plan for the financial stability of the database; funding should include gifts, grants, fees, or donations; and

 

     BE IT FURTHER RESOLVED that the Committee consists of the following members:

 

     (1)  One member of academia with experience in health care data and cost efficiency research;

 

     (2)  One representative of the Healthcare Association of Hawaii;

 

     (3)  One representative of an integrated, multi-specialty health care organization;

 

     (4)  One representative of the Hawaii Medical Association representing physicians and surgeons;

 

     (5)  One representative of small businesses, as determined by Title 13 of the Code of Federal Regulations section 121.201, that purchase group health insurance for employees; provided that the representative is not a supplier or broker of health insurance;

 

     (6)  One representative from one of the largest employers in the State that purchases health insurance for employees; provided that the representative is not a supplier or broker of health insurance;

 

     (7)  One representative of self-insured employers; provided that the representative is not a supplier or broker of health insurance;

 

     (8)  One representative of an organization that processes insurance claims or certain aspects of employee benefit plans for a separate entity;

 

     (9)  One representative of a non-profit organization with demonstrated experience working with employers to enhance value and affordability in health insurance;

 

    (10)  One person with a demonstrated record of advocating health care privacy issues on behalf of consumers;

 

    (11)  One person with a demonstrated record of advocating health care issues on behalf of consumers;

 

    (12)  One representative of the Hawaii Primary Care Association;

 

    (13)  One representative of non-profit health insurers;

 

    (14)  One representative of for-profit health insurers;

 

    (15)  One representative of pharmacists or an affiliate society;

 

    (16)  One representative of the Hawaii Health Information Corporation; and

 

    (17)  One representative of non-profit organizations that facilitate health information exchange to improve health care for the people of Hawaii; and

 

     BE IT FURTHER RESOLVED that the following act as ex officio members of the Committee:

 

     (1)  The administrator of the Med-QUEST Division or the administrator's designee;

 

     (2)  The Director of Health or the Director's designee; and

 

     (3)  One representative of the Hawaii Employer-Union Health Benefits Trust Fund; and

 

     BE IT FURTHER RESOLVED that the members of the Committee select a chairperson from among themselves, and the chairperson is requested to convene meetings; and

 

     BE IT FURTHER RESOLVED that the members of the Committee serve without compensation or reimbursement for expenses; and

 

     BE IT FURTHER RESOLVED the Committee be situated in the Department of Commerce and Consumer Affairs for administrative purposes; and

 

     BE IT FURTHER RESOLVED that the Legislative Reference Bureau is requested to assist the Committee in drafting its recommendations and proposed legislation; and

 

     BE IT FURTHER RESOLVED that the Committee is requested to submit a report of its findings and recommendations, including any implementing legislation, to the Legislature no later than twenty days before the convening of the Regular Session of 2011; and

 

     BE IT FURTHER RESOLVED that the Committee cease to exist after the adjournment sine die of the 2011 Regular Session of the Legislature; and

 

     BE IT FURTHER RESOLVED that certified copies of this Concurrent Resolution be transmitted to the Governor, the President of the Senate, the Speaker of the House of Representatives, the Chair of the House Standing Committee on Health, the Director of Health, the Director of Human Services, the Director of Commerce and Consumer Affairs, the Insurance Commissioner, and the Chief Executive Officers of the Healthcare Association of Hawaii, Hawaii Medical Association, Hawaii Primary Care Association, and Hawaii Health Information Corporation.
