Report Title: Hospital Discharge Planning; Caregiver Designation; Health Care
Description: Requires hospitals to adopt and maintain discharge policies, consistent with recent updates to federal regulations, to ensure that patients continue to receive necessary care after leaving the hospital. Effective July 1, 2017. (SD1)
